On December 10, 2025, Erie Family Health Centers recently experienced a data breach that may have exposed sensitive personal, financial, and health-related information belonging to current and former patients and employees. Notification letters were sent to affected individuals by mail on and before May 14, 2026.
Erie Family Health Centers has disclosed that 570,000 individuals were affected by the breach. The compromised information may include patient name, address, phone number, email address, Social Security Number, driver’s license/state ID number, taxpayer ID number, passport number, financial account information, payment card information, online account credentials, date of birth, medical treatment or diagnosis information, prescription information, date of service, patient ID number, patient account number, provider name, medical record number, Medicare/Medicaid number, health insurance information.
