Coos County Family Health Services is a primary care services provider based in Berlin, New Hampshire. They provide preventative services, screening, diagnosis and illness treatment, and health education. 

On July 10, 2025, Coos County Family Health Services experienced a data breach that may have exposed sensitive personal and health-related information belonging to current and former patients and employees. Notification letters were sent to affected individuals on October 9, 2025. 

Coos County Family Health Services has disclosed that 40,185 individuals were affected by the breach. The compromised information may include names, addresses, dates of birth, contact information, Social Security numbers, medical information, and medical information numbers.
