Community Health Network is a non-profit health organization located in Central Indiana. They provide comprehensive services to include primary care, specialty care, hospital and ambulatory services, surgery centers, home care, behavioral health, and employee services.
On February 26, 2025, Community Health Network experienced a data breach that may have exposed sensitive personal, medical, and health-related information belonging to current and former patients. Notification letters were sent to affected individuals on September 12, 2025.
Community Health Network has not disclosed the number of individuals affected by the data breach but have notified over 13,000 Indiana residents. The compromised information may include patient names, Social Security numbers, and private medical information.
The recent Community Health Network data breach is a serious matter that could inflict far-reaching consequences for those affected. The exposure of protected medical information can pose an even greater danger of identity theft—such as obtaining medical services in the victim’s name, which will be charged to the victim.
Current and former patients and employees of Community Health Network may face an increased risk of fraud and identity theft and are advised to closely monitor their financial accounts and personal information for any suspicious activity.
