Clement Manor is a senior living facility in Greenfield, Wisconsin. It includes 102 independent living apartments and assisted living units licensed for up to 99 residents. The Clare Suites Memory Care program offers 23 suites, and its Adult Day Services and Brain Stretch Club are licensed to serve 45 participants. The on-site Health Center provides short-term rehabilitation and long-term care for up to 61 residents.

Clement Manor recently experienced a data breach that exposed sensitive personal, financial, and health-related information belonging to current and former patients and employees. Impacted individuals will receive notification by mail.

Clement Manor has not disclosed the number of individuals affected by the breach. The compromised information includes names, addresses, dates of birth, driver’s license numbers, Social Security numbers, medical diagnoses or conditions, medication details, health insurance information, provider names, other treatment information, and financial account data.
